Bihar Seeks Private Sector Involvement in 33 Medical College Projects via PPP Model

Bihar invites private players for 33 medical – Senior health officials announced that the Bihar government is seeking stakeholder input for 33 medical college hospitals, aiming to integrate private entities into the state’s public health infrastructure through public-private partnerships (PPP). The initiative is designed to attract prominent players in healthcare and medical education, according to officials.

Project Structure and Models

The plan includes 17 new medical college hospitals as greenfield projects, with the government providing land on a 60-year lease. Additionally, 16 existing or soon-to-be-constructed institutions will be offered under the brownfield model for operational management over a 30-year concession period.

Health Secretary Kumar Ravi highlighted that the state is prepared to hand over 16 colleges with existing infrastructure or those set to be completed within a year to private operators with experience in healthcare management.

Consultation Process and Objectives

A stakeholder meeting is scheduled for June 17 in Patna to gather insights from potential investors and healthcare providers. This step precedes the finalization of project frameworks and the bidding process.

“Bihar expects participation from reputable private entities with a track record in running medical colleges and hospitals. We are ready to offer 16 institutions where infrastructure is already in place or will be completed within six to twelve months,” Ravi stated.

The consultation aims to evaluate investor interest, examine risk-sharing strategies, and identify optimal PPP structures, including design-build-finance-operate-transfer (DBFOT) models.

Refining Bidding Framework

Ravi mentioned that the government will present concept notes for both project types and solicit feedback on land needs, regulatory clearances, and mandatory certifications. A single-window mechanism for approvals is also under consideration.

“After consultations, we will study industry best practices and the Centre’s PPP guidelines to refine the bidding documents,” he added.

Following this, a transaction advisor will be appointed to draft detailed terms, such as revenue-sharing agreements, before the bidding process begins. Decisions on pricing regulations and subsidized care for economically disadvantaged patients will be made later.

Strategic Importance of PPP

Officials emphasized that the PPP approach will expedite medical college establishment, elevate service quality, and broaden access to specialized healthcare. This method also promises faster development than traditional public funding models.

The initiative aligns with Bihar’s Saat Nischay (Seven Resolves)-3 program for 2025–30, which targets expanding medical education and healthcare facilities to tackle shortages of doctors, specialists, and tertiary care services in the state.
